Important Notice Regarding "Consumers Market Survey, Inc." Financial Identity Theft Scheme

Lucile Packard Children’s Hospital (LPCH) is the target of a financial identity theft scheme.  This identity theft scheme does not involve any LPCH patient information or information pertaining to individuals who have paid for services provided by LPCH.
 
An organization identifying itself as Consumers Market Survey, Inc. is sending letters to random individuals across the United States soliciting participation in a fictitious secret shopper program.  Participants are provided a fraudulent check that appears to be from LPCH and are instructed to deposit the check and wire cash to London, United Kingdom. 
 
LPCH has no connection or agreement with Consumers Market Survey, Inc. or any other organization to conduct such activities.  Therefore, LPCH has taken all necessary precautions to mitigate this situation.  If you have received an LPCH check from Consumers Market Survey, Inc. or any other organization soliciting participation in a similar secret shopper program, please contact the LPCH Finance Office at (650) 736-8904.  Thank you.
