New Patients

We are happy to answer questions from prospective new patients. Please feel free to call us at (650) 724-2221, Monday-Friday, 7:30 am-4:30 pm.
 
Before we arrange for any Program for Placental Disorders program-related appointments, we will need your obstetrician or midwife to:

1. Complete a referral form (PDF) and fax it to (650) 723-6607, or 
2. Call us at (650) 724-2221 to make a referral.

Referring Health Care Providers

The Program for Placental Disorders coordinator is your central point of contact. Please feel free to call us any time between 7:30 am-4:30 pm at (650) 724-2221 should you have any questions or concerns about your patient.
 
Before we arrange for any appointments for your patient, we will need you to:

2. Call us at (650) 724-2221 to make a referral.

After hours consults and transport. Immediate consultation with a maternal-fetal medicine specialist or acute transport is available by calling the Lucile Packard Children’s Hospital Transport Dispatch Center at (650) 723-7342 or (800) GO-FOR-LPCH.
