To Schedule:
1. We require a written referral faxed by your physician. Please have your healthcare provider fax the completed form, along with any necessary lab results to:

2. Please call the Perinatal Diagnostic Center to make an appointment.
To Prepare:
Your partner is welcome to come to your Perinatal Diagnostic Center appointment, but please do not bring your other children.If you are having a nuchal translucency (NT) ultrasound or chorionic villus sampling (CVS), please drink 16 ounces of water 45 minutes before your procedure.
To Bring to the Appointment:
Please bring the following items to your Perinatal Diagnostic Center appointment:- Insurance card
- Photo ID
- List of any medications you are taking
After Your Appointment
The results of your patient's tests or a summary of a consult or genetic counseling meeting will be sent to you as soon as possible. Results of nuchal translucency and level 2 ultrasound will be reviewed with you by a perinatalogist at the end of the appointment.
