Motion & Gait Analysis Laboratory Information for Health Care Providers

The Motion & Gait Analysis Laboratory provides comprehensive patient gait analysis or upper limb analysis reports for orthopedists, neurologists, physiatrists, pediatricians, podiatrists, physical therapists and other specialists.

How to Refer a Patient

Please complete the following steps to refer a patient to the Motion & Gait Analysis Laboratory:

1. Complete a patient referral form:
   • Referral Request Form (PDF)
2. Include the patient’s most recent clinic note, including surgical history and any treatments under consideration. 
   
   
3. Send the information to us:

After we receive the patient referral form, we will:

1. Contact the patient to schedule an appointment
2. Obtain pre-authorization for treatment from the insurance company or California Children’s Services, if needed

Communicating with Referring Health Care Providers

Once the patient has gone through an analysis, referring health care providers will receive a:

• Paper copy of the patient report, including:
  • Test results
  • Treatment recommendations
    
    
• CD that contains:
  • Video
  • Electronic copy of the patient report
