Pediatric Pharmacy Residency Application Procedures

 
The Pediatric Pharmacy Residency Program (PGY1) application deadline is Friday, January 13, 2012. NO applications will be accepted beyond this date.

Requirements

Applicants must meet all of the following criteria:
  1. Enroll in the ASHP Residency Match Program

  2. Possess a Doctor of Pharmacy from an accredited school or college of pharmacy OR have equivalent experience

  3. Obtain licensure to practice pharmacy in the state of California before January 1, 2013: http://pharmacy.ca.gov

  4. Participate in an on-site interview at Lucile Packard Children's Hospital

  5. Possess a CA pharmacy intern license at the time of hire (or within 30 days if candidate is from out of state)

Submitting an Application

Candidates interested in applying must complete the following four steps:
 
1. Complete the Online Application
  1. Create an account
  2. Under Search by Job Number, enter 20608 and click Find Job
  3. Follow the instructions on the Web site
  4. When finished, click submit
Please attach your curriculum vitae to your online application under the resume section
 
2. Send Letters of Recommendation
Three letters of recommendation AND recommendation evaluation forms are required. Please have each recommender:

  • Send a signed hard copy of recommendation materials to:

3. Submit Transcripts  
College of Pharmacy transcript should be sent directly from your school to:

4. Submit Personal Statement and Curriculum Vitae
Send your personal statement outlining career goals, areas of interest and reasons for applying to the residency program at Lucile Packard Children’s Hospital to:

On-Site Interview

After review of application materials by the residency committee, selected applicants will be contacted for a three hour on-site interview in January–March 2012.  For consideration, the online application and all supporting documentation must be received by January 13, 2012.
