General Care Unit Medical Urgent Admissions (Pediatric)

You may first page the on-call nursing supervisor at (650) 723-8222 ID# 18672 or call the Physician Referral Line at (650) 497-8221 or (800) 995-5724. The physician line staff will assist you in facilitating the admission and direct you appropriately.

You will be asked to provide the following information during the admission process:

• Your name, phone number, and pager number
• Patient's name, address, and phone number
• LPCH medical record number (if known)
• Date of birth
• Diagnosis
• Procedure(s)
• Estimated length of stay
• Attending/admitting physician
• Special room request
• Mode of transportation
• Insurance information

The admitting staff will be notified of the expected admission..

All community physicians and all faculty physicians not participating in the pre-certification program are required to obtain necessary authorizations and certifications for the admission.
 
The admitting staff will contact the physician’s office if there are problems or services have been denied by the insurance company.
