Intensive Care, Emergent or Urgent Admission

You may first page the on-call nursing supervisor at (650) 723-8222 ID# 18672 or call the Physician Referral Line at (650) 497-8221 or (800) 995-5724. The physician line staff will assist you in facilitating the admission and direct you appropriately.

Please have the following information available when calling in your admission:

• Patient's name
• LPCH medical record number (if known)
• Date of birth
• Diagnosis
• Procedure(s)
• Admitting physician
• Patient's present location
• Estimated time of arrival or timeframe for effecting transfer
• Insurance information
  

The Admitting Department will be notified of your patient's pending admission.

Parents/responsible parties will be asked to come to Admitting to sign required admission documents.
